C-3
Description
C-3 is a broad spectrum cephalosporin antibiotic of third generation for oral administration. It is a bactericidal antibiotic. C-3 is highly active against Neisseria gonorrheae, Haemophilusinfluenzae, Moraxella catarrhalis including betalactamase producers, most of the Enterobacteriaceae, beta-haemolytic Streptococci (group A & B) and Streptococcus pneumoniae. C-3 is more active than other oral cephalosporins against Escherichia coli, Klebsiella spp., Proteus mirabilis and Serratiamarcescens. C-3 is also active against Streptococcus pyogenes. 40-50% of an oral dose is absorbed from gastro-intestinal tract, whether taken with meals or not. The plasma half-life is usually about 3 to 4 hours and may be prolonged when there is renal impairment.

Composition
C-3 200 Capsule: Each capsule contains Cefixime trihydrate BP equivalent to Cefixime 200 mg.
C-3 400 Capsule: Each capsule contains Cefixime trihydrate BP equivalent to Cefixime 400 mg.
C-3 Powder for Suspension: Each 5 ml reconstituted suspension contains Cefiximetrihydrate BP equivalent to Cefixime 100 mg.

Indication
Upper and lower respiratory tract infections, Urinary tract infections and Gonococcal urethritis, Typhoid fever.

Dosage and Administration
C-3 Capsule:
Adult Dose: The recommended dose of C-3 is 400 mg (Cefixime BP 400 mg) daily. When necessary 1 or 2 capsules (200-400 mg) as a single dose or in 2 divided doses daily. Typhoid Fever: 15-20 mg/kg/day in two divided doses for 7-14 days.
C-3 Suspension:
Child Dose: 8 mg/kg daily as a single dose or in two divided doses as 4 mg/kg every 12 hours. Typhoid fever: 15-20 mg/kg/day in two divided doses for 7-14 days; not to exceed 400 mg/day.

Side Effect
C-3 is generally well tolerated. The majority of adverse reactions observed in clinical trials were mild and self-limiting in nature. Gastro-intestinal disturbances: Diarrhoea (if severe diarrhoea occurs C-3 should be discontinued), changes in the colour of stool, nausea, abdominal pain, dyspepsia, vomiting and flatulence have been reported. CNS disturbances: Headache and dizziness. Others: Hypersensitivity reactions which usually subside upon discontinuation of therapy infrequent and reversible haematological changes; elevation of serum amylase.

Contraindication
Patients with known hypersensitivity to Cefixime or cephalosporin group of drugs are contraindicated to this drug.

Drug Interaction
C-3 should be administered with caution to patients receiving anticoagulants such as warfarin potassium. Since C-3 may enhance effects of the anticoagulants, prolonged prothrombin time with or without bleeding may occur.

Precaution
C-3 should be prescribed with caution in individuals with a history of gastrointestinal diseases, particularly colitis. Dosage adjustment is only necessary in severe renal failure (creatinine clearance < 20 ml. min-1).

Use in Pregnancy and Lactation
Pregnancy category B. It is probably best to avoid using the drug during pregnancy, and by the nursing mother.
Direction for Reconstitution of Suspension

Shake the bottle to loosen powder. Add 30 ml boiled and cooled water. For ease of preparation, add water to the bottle in two proportions. Shake well after each addition until all the powder is in suspensions. The reconstituted suspension should be used within 7 days if kept in room temperature or within 14 days if refrigerated. Shake well before using the suspension.

Storage
Protect from light, store in a cool and dry place, Keep out of reach of children.

C-3 400 Capsule: Box containing 2 X 4 capsule in Alu-Alu blister pack.
C-3 Powder for Suspension: Bottle containing dry powder for 50 ml suspension.
